29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: Rock crusher: On or about May 2, 2017, employees operated a rock crusher during the recycling of concrete pieces and an employee entered the crusher hopper prior to shutting down using procedures established for the machine or equipment. This exposed the employee caught in and struck by hazards.
