29 CFR 1910.119(f)(1)(i)(G): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address the startup following a turnaround, or after an emergency shutdown: The employer is failing to protect employees from explosion and fire hazards by failing to address and develop startup procedures following an emergency shutdown as part of the Standard Operating Procedures. This was most recently documented at 25201 East 78 Highway, Independence, Missouri, 85, bay 901.The company developed a Tetrazene Standard Operating Procedure, SOP Number 85A-11-2, which does not contain procedures for startup following an emergency shutdown.
