29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: (a) Employees were exposed to amputation hazards from the 500 Line Vicon Machinery in B Side Fittings. The rotating shaft near the end of the machine where the belt and pulleys are connected was not adequately guarded. On or about February 3, 2017, an employee received an amputation when his hand got caught in the rotating shaft, and belt and pulley at the end of the machine. (b) Employees were exposed to amputation hazards from the 500 Line Vicon Machinery in B Side Fittings. The rotating shaft at the very end of the machine that forms the collars was not adequately guarded. The outgoing end is unguarded. (c) Employees were exposed to amputation hazards from rotating shafts on the Old Collar Line Roller Feed Assembly in B side Fittings. The rotating shafts had a guard which was inadequate. There was an opening in the side exposing employees to the rotating shafts. (d) Employees were exposed to amputation hazards from rotating shafts on the Shear to Length Line. There were ingoing nippoints from the rotating shafts at the beginning of the machine after the decoiler, and at the end of the machine. There were no guards to protect employees from the ingoing nippoints.
