29 CFR 1910.147(f)(4): Specific procedures were not utilized during shift or personnel changes to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout device protection between off-going and oncoming employees, to minimize exposure to hazards from the unexpected energization or start-up of the machine or equipment, or the release of stored energy: (a) On or about February 15, 2017 and at times prior thereto, the employer did not develop or utilize specific procedures for the orderly transferring of lockout devices during shift/personnel change, exposing employees to hazards associated with the unexpected release of hazardous energy.
