29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: Tire Shredder Machine Area: Employer did not provide protective helmets to employees who work on or near the tire shredder machine area such as operating the conveyors, clearing jams in the chute and conveyors, feeding tires onto the conveyors and shoveling debris off the floor and runways and were exposed to falling objects such as tire parts and tools used to clear jams and maintenance.
