29 CFR 1926.1211(c)(2): The employer did not ensure that each authorized entrant had the other end of the retrieval their line attached to a mechanical device or fixed point outside the permit space in such a manner that rescue can begin as soon as the rescuer becomes aware that rescue is necessary: a. E E Robinson Park (Level Creek), 800 Peachtree Industrial Blvd. Sugar Hill, GA - An employee was exposed to asphyxiation hazard. The employer did not ensure that the entrant had a retrieval line attached to a mechanical device or fixed point outside the space to ensure rescue could begin as soon as necessary. The employer remove the manhole cover and purchased a flash light with the expectation that the employee would break the plain, on or about September 6, 2016.
