29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval: Cold flash area, Wyman Gordon Company, 244 Worcester Street, North Grafton, Massachusetts; The left fork on forklift 9663 in the Cold Flash Area was modified without authorization from the manufacturer. A piece of steel had been wrapped around the fork to keep it in place during operation. This could allow for the shifting of the forks which could cause the stock to fall and strike an employee exposing them to struck by hazards.
