29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): A) Fresh Unlimited, Inc. DBA Freshway Foods, located at 601 North Stolle Avenue, Sidney, OH 45365: On or about February 1, 2016, the employer did not ensure that the point of operation of the 3M-Matic Case Sealing Machine, located on the Tomato Line, was adequately guarded. One of the machine operators sustained an injury to the right hand after being hit by the machine's label arm. Employees were exposed to struck-by hazards.
