On or about October 2, 2015, and times prior to, at the MEMC Pasadena, Inc. facility in Pasadena, TX, the employer did not ensure that specific requirements for testing the effectiveness of the isolation of the K92350 Silane compressor before employees began blinding the system for maintenance.
