29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. (a) At the asphalt silo, on or about August 5, 2015 and at times prior to, the procedure used by an employee to remove an energy isolating device from the silo retention gates did not outline the safe removal of the device exposing the employee to a caught-in and struck-by hazard.
