29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a.) On or about January 7, 2015, at the above addressed jobsite, the employer did not ensure that locks and tags were used by authorized employees to secure electrical power at disconnect panels on two baling machines that employees had been maintaining and had not de energized and locked the electrical disconnect panel before maintaining or clearing jams.
