29 CFR 1910.333(b)(2)(ii)(A): Safe procedures for deenergizing circuits and equipment shall be determined before circuits or equipment are deenergized. The employer had not developed a safe procedure for deenergizing circuits including the middle switch blade on the electrical switchgear within a DP&L/Metal Shredders, Inc. 4.16kv dual-access substation that was stuck in the closed position before entering the substation on October 16, 2014 that included procedures such as but not limited to; the use of PPE for the arc flash hazard and how to test the electrical lines with a voltage detector. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
