29 CFR 1910.134(c)(1)(iv): The written program did not contain procedures for the use of respirators in routine and reasonably foreseeable emergency situations: (This standard is incorporated by Construction Reference 29 CFR 1926.103). (a) Georgetown Wastewater Treatment Lagoon: On August 28, 2014, an employee entered a confined space (5' x 5' x 18.5'), through a 2 foot diameter manhole, to perform the spray application of Flex Seal sealant and the atmosphere became immediately dangerous to life and health (IDLH). No emergency rescue respirators or supplied air respirators were available for use at the site for emergency use. (b) Georgetown Wastewater Treatment Lagoon: On or about August 28, 2014, the supervisor attempted to rescue the victim when the atmosphere was immediately dangerous to life and health (IDLH). No emergency rescue respirators or supplied air respirators were available for use at the site for emergency use. NOTE: Abatement certification and documentation are required for this item.
