29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about December 13, 2013, at Cajun Sugar in New Iberia, LA; the employer did not: a) Isolate the Crusher Donnely Chute and/or mill roller from all energy sources; b) Apply a lockout or tagout device to secure the Crusher Donnely Chute and/or mill roller from all energy sources; and/or c) Verify isolation of the Crusher Donnely Chute and/or mill roller from all energy sources.
