Beech St., Long Beach NY, North Side - Employee was installing tar paper and asphalt shingle on the roof of a two story residential home. The employee was not protected from falling approximately 25ft to the ground below; on or about 11/5/13. b) Worksite, 716 W. Beech St., Long Beach NY, South Side - Employee was installing asphalt shingle on the roof of a two story residential home. The employee was not protected from falling approximately 25ft to the ground below; on or about 11/5/13. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
