29 CFR 1910.215(a)(4): Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage: a) Die Shop: The work rest on a Delta bench grinder was adjusted so that the gap between the abrasive wheel and the work rest was approximately 1 inch exposing workers to a struck-by hazard, on or about July 10, 2013. ABATEMENT CERTIFICATION REQUIRED
