29 CFR 1910.147(c)(6)(ii): The employer did not certify that the periodic inspections have been performed. The certification did not identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection: (a) On or about June 24, 2013, and at times prior thereto, the employer did not conduct or certify that management had observed employees performing lock out or tag out procedures. Note: Abatement certification and documentation ARE required for this item.
