29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Machine Shop, on or about 3/29/12: Walker Turner drill press was not provided with a chuck guard. b) Near A14 Vertical CNC Mill, on or about 3/29/12: Procut drill press, Model No. RL 79DPF12343, was not provided with a chuck guard. c) Near A6 Matsuura Horizontal CNC Mill, on or about 3/29/12: Drill press was not provided with a chuck guard. d) Vending Area, on or about 3/29/12: (3) Three Bridgeport Mills were not provided with chuck guards. e) Vending Area, on or about 3/29/12: Acer Mill was not provided with a chuck guard. f) Drill Press Area, on or about 3/29/12: Cincinnati multi-drill table was not provided with chuck guarding. g) Drill Press Area, on or about 3/29/12: Buffalo drill press was not provided with a chuck guard. h) Drill Press Area, on or about 3/29/12: (2) Two Delta multi-drill tables were not provided with chuck guarding. i) Near Matsuura Horizontal CNC #10, on or about 3/29/12: Walker Turner drill press was not provided with a chuck guard. j) CNC Lathe Area, on or about 3/29/12: Hardinge Lathe was not provided with a chuck guard. k) Fastener Assembly Area, on or about 3/29/12: Delta drill press was not provided with a chuck guard. l) Maintenance, on or about 4/4/12: Ridgid Pipe Threader, activated by a manually operated on/off switch that would not automatically shut the machine off should the operator be inadvertently caught up in the rotating parts.
