29 CFR 1910.147 (f)(3)(ii)(D) Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section. a) Polyester plant: Written procedures for lockout tagout did not include such information as, but not limited to, the specific energy sources, the specific methods to control the energy sources,the sequence of activities required to control the hazards, the location of the isolation devices used, how is stored energy in the system released, the necessity for air monitoring equipment and methods to test the system to determine de-energization, on or about 10/12/11. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
