29 CFR 1910.119(d)(3)(ii): The employer did not document that the equipment in the process complied with recognized and generally accepted good engineering practices: a) On or about February 24, 2010 - the ammonia refrigeration machine room did not have an eye wash and safety shower quick drenching unit located external to the machine room and readily accessible to employees that were exposed to ammonia liquid or vapor as specified in ANSI/IIAR Standard 2-2008, Section 13.3.1.4. and IIAR Bulletin 109, Section 4.10.10, 1997 edition.
