29 CFR 1926.1413(b)(4): The wire rope inspections were not documented according to Sec. 1926.1412(e)(3) (monthly inspection documentation): On or about June 24, 2025, and at times prior thereto, at the Massey site, the employer did not ensure certified monthly wire rope inspections were documented for the wire ropes utilized by employees maneuvering, demolishing, and cleaning up the unexpected spaceship anomaly debris.
