29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Jobsite located at or near 5800 City Ave, Philadelphia PA- On or about June 24, 2025, the employer did not train each employee conducting masonry activities on scaffolds, who were exposed to fall hazards of approximately 60 feet, to recognize the hazards associated with working from scaffolds that had, missing planking and guardrails.
