29 CFR 1926.1203(e)(2)(iii):Before an employee enters the space, the internal atmosphere must be tested with a calibrated, direct-reading instrument, for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants, in that order: On or about April 22, 2025, in Section B- Subfloor, employees entered a confined space area and sprayed foam insulation containing isocyanates, and there was no internal atmosphere in the space, exposing the employees to hazards such as, but not limited to, flammable gases, oxygen deficient atmosphere, or toxic air contaminants.
