29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: On or about 2/7/2025, the abrasive wheel on the company's Dayton Bench Grinder (Model #: 49H004A) was worn leaving approximately 2-inches between the wheel and the work rest. The operator and/other employees working in the immediate area were potentially exposed to flying projectiles and/or shards of the abrasive wheel and other materials if the wheel were to break or material got caught between the frame and work rest.
