29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control. (a) In the kettle room - On or about October 2, 2024, the employer exposed employees to struck-by hazards, in that, the energy control procedures for shutdown of equipment associated with the heat exchangers did not clearly and specifically outline the steps to verify the effectiveness of lockout devices or release of stored energy within the system.
