This condition exposed employees to lacerations and amputation hazards. (b)Rapid Packaging Company, Inc. at 1310 West 7th Avenue, Brookings, SD 57006: On or about and at times prior to January 7, 2025, the employer did not ensure adequate guarding methods were in place and in good working condition on the General Gluer, Serial# L5095. This condition exposed employees to laceration and amputation hazards. (c)Rapid Packaging Company, Inc. at 1310 West 7th Avenue, Brookings, SD 57006: On or about and at times prior to January 7, 2025, the employer did not ensure adequate guarding methods were in place on the Bar Slitter Model: RJE 120" 4.
