29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. (a) On or about August 15, 2024, the employer did not ensure that each doughnut maker, baker, and operator was adequately trained on the methods and means necessary for the isolation and control of all hazardous energy sources prior to starting work. Employees were performing servicing and maintenance tasks such as, but not limited to, fixing trays, cleaning, and unjamming on doughnut production machines (e.g. Krispy Kreme 270 PX Line including Extruder/Proofer). Employees were exposed to caught-in hazards of the unexpectedly moving parts including trays, and cutters likely to result in serious injuries because they did not know when lockout was required, what energy sources needed to be isolated, and how to control and verify isolation of those energies. (b) On or about August 15, 2024, the employer did not ensure that each doughnut maker, baker, and operator was adequately trained on the methods and means necessary for the isolation and control of all hazardous energy sources prior to starting work. Employees were performing servicing and maintenance tasks such as, but not limited to, cleaning, and unjamming on doughnut production machines (e.g. Magna Barrel Mixer). Employees were exposed to caught-in hazards of the unexpectedly moving parts including a rotating barrel likely to result in serious injuries because they did not know when lockout was required, what energy sources needed to be isolated, and how to control and verify isolation of those energies. (c) On about August 15, 2024, the employer did not ensure that each sanitation worker was adequately trained on the methods and means necessary for the isolation and control of all hazardous energy sources prior to starting sanitation work. Employees were cleaning and sanitizing energized Krispy Kreme 270 PX doughnut production line from the extruder to the packing tables. Employees were exposed to caught-in hazards of the moving/rotating parts likely to result in serious injuries because they did not know when lockout was required, what energy sources needed to be isolated, and how to control and verify isolation of those energies.
