29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) 731 Avenue H Columbia, PA 17512 and 615 Locust St. Columbia, PA 17512 - Facility Wide - On or about August 8, 2024, employees were conducting service and maintenance of machinery used for glass etching and painting including, but not limited to, laser engravers, CrystalBlast 3624 sandcarving equipment, and Pad Printer. The employer did not have a hazardous energy control program with elements including, but not limited to, procedure(s), employee training, and periodic inspections. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
