29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment. On or about February 2, 2024, the employer failed to install a door and cover holes, knockouts, and openings for the breaker box that supplied electricity to the Automatic Line. The lack of covering for the box exposed maintenance workers and employees using the timeclock next to the breaker box, electrical hazards up to 120 volts.
