Mingo's Construction LLC located at 860 Jaeger Street, Columbus, OH 43206: On or about October 4, 2023, during the construction of a mixed-use (commercial/residential) building, employees were engaged in the rigging of building materials such as bundles of OSB sheathing and hoisting the material with the use of a tower crane to the second deck. The employer did not ensure the materials being lifted were rigged to prevent unintentional displacement of the load while it was within the fall zone. The following condition exposed employees to struck-by hazards.
