29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about May 23, 2023, at the Prime G Reboiler Pass 4 Control Valve, Wynnewood Refinery in Wynnewood Oklahoma, employees did not affix lockout or tagout devices to each energy isolating device prior to mechanical disassembly of the valve. This exposed the employees to fire and chemical release hazards.
