29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: a) Lots 505/506 - The employer did not ensure the elevated platform utilized by employees installing roof sheathing was firmly secured to the quick coupler assembly on the boom of a Caterpillar TH460B telehandler. The quick coupler was not completely engaged onto the hooks of the platform and the locking pin and retaining clip were not utilized, on or about March 17, 2023. No abatement�certification or�documentation required.
