29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): Employees engaged in maintenance/housekeeping activities in the equipment room of the facility were exposed to amputation and caught-in hazards. The drive pulleys on the double stack pumping station units were not provided with guards on the back of the machine. In-going nip points were created by two exposed 14 inch pulleys and associated belts on the back of the machine.
