29 CFR 1910.119(f)(1)(ii)(B): The employer did not develop and implement written operating procedures that provide clear instructions for steps required to correct or avoid deviation from the operating limits in the covered process. (a) Room 30 HEPES process - On or about 11/9/2022, the employer operates and maintains a covered process involving methanol, a class 1 flammable liquid having a flashpoint below 100F exceeding the TQ of 10,000 lbs. The employer did not develop and implement written operating procedures, including the SAN WI 235 Loading HEPES Crystallizer T-30.03 with Methanol (effective date 9/01/2022) procedure, that provided clear instructions for steps required to correct or avoid deviation from the operating limits, in this case where the overfilling of the crystallizer were to occur.
