29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Polishing Room - The two G&P Machinery model SS-1000 double end spindle stands (serial numbers G-13423 and G-13738), equipped with polishing wheels on both sides on both machines, did not have guarding on the polishing wheels nor the spindle ends, exposing employees to entanglement hazards from the rotating polishing wheels and spindle ends, on or about October 26, 2022. (b) Grind - Department 3 - The two Admiral double end spindle stand abrasive belt grinders with floor level back stand idlers did not have guarding on the top pulleys, on the top pulley spindle ends, on the top run of the abrasive belts, nor on the back stand pulleys and spindle ends, exposing employees to in-going nip point and entanglement hazards, on or about October 27, 2022. (c) Cut, Bend, Form - Department 1 - The Lodge & Shipley Company shear, model 0308-AB, serial number 46918, hand-fed and actuated by foot pedal to cut aluminum sheets, had 3/8" high openings under the hold-downs, exposing employees to the hazard of getting fingers struck-by the hold-downs, on or about October 28, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
