29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about August 19, 2022, at an establishment located at 3555 Moser Street in Oshkosh, Wisconsin; employees utilizing the Lift Products Max-Tilt Hydraulic Tilt Upender LPTT-UP-30-90 were exposed to crushing hazards created by the rotating movement of the tilting mechanism and the material platform. The machine was not effectively guarded to prevent exposure to this hazard. b) On or about August 19, 2022, At an establishment located at 3555 Moser Street in Oshkosh, Wisconsin; Extrusion Operator employees on the Welex SL6 Winder were exposed to entanglement and caught-in hazards created by the rotating shaft, the core, and the plastic sheet. The machine was not effectively guarded to prevent exposure to these hazards on both the platform on the winder and on the outfeed side of the winder. c) On or about August 19, 2022, at an establishment located at 3555 Moser Street in Oshkosh, Wisconsin; Material Handler employees in the area of the Core Cutter #1 were exposed to caught-in hazards created by the unguarded chain and sprocket, and the belt and pulley on the drive motor that powers the spindle and core pusher. The machine was not effectively guarded to prevent exposure to this hazard. d) On or about August 19, 2022, at an establishment located at 3555 Moser Street in Oshkosh, Wisconsin; Material Handler employees operating the Core Cutter #1 were exposed to caught-in and pinch point hazards created by the unguarded rollers which resulted in an ingoing nip point when the cutting blade contacted the core being cut. The machine was not effectively guarded to prevent exposure to these hazards. e) On or about August 19, 2022, at an establishment located at 3555 Moser Street in Oshkosh, Wisconsin; Material Handler employees operating the Core Cutter #2 were exposed to caught-in and pinch point hazards created by the unguarded rollers which resulted in an ingoing nip point when the cutting blade contacted the core being cut. The machine was not effectively guarded to prevent exposure to these hazards. f) On or about October 30, 2022, At an establishment located at 3555 Moser Street in Oshkosh, Wisconsin; Extrusion Operator employees on the SL3 Winder were exposed to entanglement and caught-in hazards created by the rotating shaft, the core, and the plastic sheet. The machine was not effectively guarded to prevent exposure to these hazards on both the platform on the winder and on the outfeed side of the winder.
