29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person. a: Market View Apartments, Carpenters Circle, Ithaca NY, 14850, on or about 6/3/22: Workers used anchorages that were not according to the design of the equipment, lifelines passed over a truss and connected back onto itself, and systems that would ensure contact with the lower level if a fall occurred. No observed system was adequate, and all were utilized under the oversight of the superintendent. Abatement certification must be submitted for this item.
