1910.120(q)(3)(v) The individual in charge of the ICS did not limit the number of emergency response personnel at the emergency site, in those areas of potential or actual exposure to incident or site hazards, to those who are actively performing emergency operations. However, operations in hazardous areas shall be performed using the buddy system in groups of two or more. On or about December 23, 2021, employees at ExxonMobil Refinery in Baytown, Texas, not all responding employee personnel used the buddy system to be accompanied by ERT personnel when entering the unit during an emergency to isolate/close valves in order to block-in fuel after certain remote operated valves became inoperable as a result of a fire.
