29 CFR 1910.146(c)(4):When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: (a) CJ TMI Manufacturing America, LLC, waste water pit: Maintenance Mechanics are required to enter a waste water pit to collect water samples and/or fix malfunctioning equipment were potentially exposed to atmospheric, slip, and fall hazards. The employer failed to have a written permit space entry program, on or about 11/24/2021. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
