29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) Jobsite located at 2001 Beach Street, Philadelphia, PA- The employer did not ensure that the job-made platform used to elevate employees was firmly secured to a JLG all-terrain forklift. Employee was working from the job-made platform which was elevated approximately 30 feet from the ground; thus exposing the employee to fall hazards. Condition was observed on or about September 8, 2021.
