29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). On or about July 27, 2021 in the Acetyls Unit of the LyondellBasell chemical complex in La Porte, Texas, workers were exposed to process materials, toxic release, and flammable hazard while performing service work that includes removing components of HV-117 when an assessment was not performed to determine if hazards were present, or likely to be present, and workers were not required to use the types of PPE necessary to protect them from the hazards that were present.
