29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: a) Throughout the warehouse, on or about July 1, 2021, the employer failed to ensure a hazard assessment was completed to determine what type and size body harnesses were needed for employees operating Powered Industrial Vehicles (order pickers), thereby exposing employees to hazards associated with falls from up to 30 feet.
