29 CFR 1910.215(a)(4):Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: In the Fabrication Maintenance Shop, employees engaged in maintenance activities were exposed to struck by hazards in that the work rest for the Powergrind pedestal grinder was not adjusted to one eighth of an inch, and was used with an opening 1.5 inches.
