29 CFR 1910.146(d)(3)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including specifying acceptable entry conditions: a - On or about January 28, 2021, at the worksite Foundation Food Group, Inc., located at 2076 Memorial Park Drive, Gainesville, GA, at Plant 4, in the Line 2 and Line 4 liquid nitrogen freezers, employees were exposed to mechanical, thermal and asphyxiation hazards when entering permit required confined space areas within the liquid nitrogen freezers as the employer did not implement adequate procedures for entry including specifying acceptable conditions including, but not limited to atmospheric conditions.
