29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation: The employer is failing to protect employees from exposure to electrical hazards while handling extension cords. This was most recently observed on May 13, 2020, at 530 Carl St, Butte, Nebraska, where an extension cords outer insulation was pulled away exposing its inner wires.
