29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. On or about March 30, 2020, at the trench jobsite, the employees working in a trench that measured 8 feet 4 inches deep were exposed to crushing and entrapment hazards, in that, they were not trained on the hazards associated with working in a trench. b.
