29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Duo Matic Machine - On or prior to December 13, 2019, a guard was missing/not installed on the non-operator side of the Duo Matic Machine (The Duo Matic Assembly System is manufactured by Viking Engineering and Development, Patent #3,945,549. The machine label also includes 950-30-1147) potentially exposing employees to caught-in hazards. b) Duo Matic Machine - On or about October 8, 2019, a guard was missing/not installed on the operator side of the Duo Matic Machine (The Duo Matic Assembly System is manufactured by Viking Engineering and Development, Patent #3,945,549. The machine label also includes 950-30-1147) exposing employees to caught-in hazards.
