29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): a. On or about 12/12/2019, at the fabrication area shear line 1, employees were exposed to a struck by hazard, in that, a series of pulleys at the side of the shear line table did not have a guard to fully enclose the moving parts. b. On or about 12/12/2019, at the fabrication area shear line 2, employees were exposed to a struck by hazard, in that, two pulleys at the side of the shear line table below the point of operation guard did not have a guard to fully enclose the moving parts. c. On or about 12/12/2019, at the fabrication area shear line 2, employees were exposed to a struck by hazard, in that, a series of pulleys at the side of the shear line table did not have a guard to fully enclose the moving parts.
