29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. Facility - On or about November 19, 2019, the employer failed to implement and enforce the use of an adequate emergency control program. The employer did not have a program which specifically outlined steps for each piece of machinery that the employees were assigned their lockout tag out duties. The employer did not have a program that included periodic inspections, specific procedures for locating, shutting down, isolating, blocking and verification of all deenergized energy sources to protect employees from hazards associated with lockout tag out.
