29 CFR 1910.119(j)(4)(i): Inspections and tests were not performed on process equipment. On or about November 3, 2019, and at times prior thereto, at Dow Chemical Company Glycol #2 facility in Plaquemine, Louisiana, the employer did not ensure PSM critical process piping components, including but not limited to, check valves were inspected and maintained to ensure its reliability and capability to prevent reverse flow.
